As the NYPD tries to piece together exactly what happened in the death of 46-year-old actor Philip Seymour Hoffman, the scene outside the late Oscar winner's West Village apartment in downtown New York City is chaotic and crowded.

The street outside of the home, located at 35 Bethune Street, is
crowded with fans, reporters, and investigators. An eyewitness tells Us
Weekly that at 3 p.m. on Sunday, Feb. 1 medical examiners and detectives
arrived on the scene.

According to the NYPD, a needle was found in Hoffman's arm from an
apparent heroin overdose, after someone -- David Katz, according to some
reports -- discovered Hoffman's body around 11:30 a.m.
The seasoned actor is survived by his three children, daughters Tallulah and Willa and son Cooper, and longtime girlfriend, costume designer Mimi O'Donnell. The distinguished actor completed a 10-day rehab stint in May 2013 after a relapse following 23 years of sobriety.
